With name, passport number & expiration date, and photo included(Photocopy of valid passport identification page. Passport must be an ordinary passport and be valid for at least six months. Applicants currently residing in China must include a copy of the page containing a valid visa or residence permit.)

Please take it to hospital to have a checkup with all items, including personal information, photo, checking date, doctors' suggestion and signature.

-Download the template, print it, fill it and ask your financial guarantor to sign and send scan copy .
-It has to be filled in English or Chinese.

*9. Letter of commitment

-Download the specific template required by the university from CUCAS.
-Fill it on computer, print it, sign, and provide a scanned copy of the signed document.
-The document must be in English or Chinese.

*10. A Self-introduction Video

-Please speak in English or Chinese.
-The video should be at least 2 minutes long, in MP4 format, and less than 7 MB.
-Include a brief self-introduction, your study plan after enrollment, reasons for applying to this program and university in China, and your plans after graduation.
-You may also highlight your language ability, interests, confidence, and motivation.

11. Study Certificate in China

-If you ever study in China, download the form, print it and ask your previous university in China to issue it with Study Certificate and Evaluation(attendance and performance).

Tips:
For applicants who are in China: If you are an employee, you still need to provide employment separation certificate and your current residence permit. If you are a student, you need to provide transfer certificate, leaving certificate, certificate of attendance rate issued by previous school and your current residence permit.
